Child Behavior Observation Map (7-17 years)

Have the following behaviors been observed in the child recently? Answer "YES" or "NO" to each item.

1. Sleep problems (frequent awakenings, night terrors and nightmares, difficulty falling asleep), loss of appetite, apathy.

2. Tendency to be alone, self-isolation, alienation, secretiveness, change in social circle (little contact with old friends and a lot with new, "virtual" ones, often older... refuses to discuss and, moreover, introduce them to parents).

3. Manifestations of aggression, agitated aggressive state of hostility (verbal, conflicts in the family, at school...; expression of physical aggression directed at others (fights) or at oneself (self-harming actions - skin cuts...); gloomy photos, blood, violence on social media).

4. Alcohol or drug abuse.

5. Abrupt changes in emotional state (suddenly increased anxiety, excessive withdrawal, depressive mood... euphoria after depression, sharp mood swings).

6. Expression of suicidal attitudes and thoughts (main themes... become death, suicide, self-torture... indifference to previously loved leisure activities; talks about death, suicide notes... expressing thoughts of one's own uselessness, meaninglessness of existence...).

7. Changes in daily activity (sharp decline in academic performance, unmotivated absenteeism, running away from home, sharp changes in dress, defiant behavior, neglect of one's appearance, failure to observe hygiene skills).

8. The child is in a group that distributes suspicious or negative content on the topics of smoking, alcohol, drugs... hatred... violence, cruelty, committing suicide, involvement in illegal activities...

9. Internet addiction. The child spends so much time on a social network or playing computer games that they even neglect daily activities: eating, sleeping, studying, which leads to the signs indicated in points 1, 2, 7.
